Many serious accidents have occurred because a machine was started when it should have been completely turned off for maintenance or repair. These accidents are usually caused by energy that hasn't been released correctly. Lock-out-tag-out (LOTO), which ensures that machines are completely off until a person with authorization releases them, helps avoid these incidents.
The LOTO system consists of a set of locks, tags and other devices that are used to isolate, secure or block the machine from an electrical source. All "affected" and "authorized" personnel are required to be trained in the use of these lockout/tag-out devices. These devices include but aren't only chains, locks padlocks and wedges, adapter pins, and self-locking fasteners. Each device is singularly identified and labeled, including the date and time of application along with the name of the authorized individual and the reason for the lockout/tag-out (repair, maintenance, etc). It must be stated clearly that these devices are not able to be removed until a complete LOTO procedure has been carried out.
